Symptomatic Tarlov cyst
Abstract
Introduction: Tarlov cysts or perineural cysts are meningeal dilatation arising between the layer of perineurium and endoneurium near the dorsal root ganglion. Most often affect sacral region and usually asymptomatic.
Case Presentation: A 53-year-old woman presented with gradually progressive low back pain with radiation to right leg. Initially it was relieved by non steroidal anti-inflammatory drugs (NSAID) but now not relieved. Examination revealed straight leg raising was limited on the right side, absent ankle jerk on the right side and hypoesthesia along the right S1 and S2 dermatomes. The Magnetic resonance imaging (MRI) of the spine revealed oval shaped CSF signal intensity cyst at the level of S1 and S2. She was
treated by laminectomy, disintegration of cyst and duroplasty. Patient reported pain relief immediately after the surgery.
Conclusion: Tarlov cysts can be a rare cause of lumbosacral radiculopathy. They must be considered in the differential diagnosis of radicular leg pain and appropriately treated by cyst excision.
