Abstract
Combined central and peripheral demyelination neuropathy, although rare, is being increasingly described in case reports. It is now a well-recognized disease spectrum. Multiple serological markers were studied for their role in diagnosis and prognosis. I present a case of CCPD, with acute myelitis and multifocal motor neuropathy with conduction block. The patient had transiently positive serum MOG antibodies. He responded well to a combination of high-dose steroids and IVIG. I propose MOG IgG1 as a potential marker that may be associated with CCPD and may aid in predicting prognosis in these patients. I recommend testing for MOG IgG1 in patients presenting with radiological and neurophysiological features suggestive of inflammatory demyelinating disorders including CCPD.
